Dave makes an outstanding point: transparency demands ruleset changes, specifically, forgiveness. If "one and done" rules apply in an environment in which "mistakes" are inevitable, we'll encourage the very things we're trying to change: behind-closed-doors rather than out-front conversations.
How do we get to forgiveness from "gotcha?" By demonstrating the attitudes we're advocating: basic charity for our fellows, one of the simplest, yet most difficult challenges any of us can face.
